REQUEST FOR RETURN GOODS AUTHORIZATION NUMBER
* ALFA International MUST approve ALL returns PRIOR to shipping any item(s) back to ALFA. We will
only issue a Return Authorization Number (RA#) once we have reviewed & approved your request.
* You MUST include a copy of your invoice or the invoice # as proof of your purchase for all goods
associated with your Request for Return.
* ALL return requests MUST be made within 30 days of invoice purchase date, UNLESS the product
Is still under warranty and your return is based on a product defect claim. Return Authorization Numbers
expire 30 days after RA issuance.
* To obtain an RA, contact ALFA Customer Service or fill out this form completely, including signature, and
email or fax the form back to us ASAP so we may process it promptly. Incomplete forms WILL NOT be

NOTE: Please read the following terms & conditions. Failure to adhere to these instructions will VOID all returns.
* Clearly mark the outside of your shipment & any related documents with your RA#. A copy of this form
should be placed inside of your shipment. RETURNS WITHOUT an ALFA issued RA# WILL NOT be
ACCEPTED under Any circumstances and returned to the sender at the sender’s cost.
* For any unused product to be considered for return, it MUST be DEEMED SALEABLE as NEW & be
in its original carton/packing to qualify for full credit. Products that have been used MUST be CLEANED
& FREE of ALL WASTE and ANY COLLATERAL MATTER before shipping. Returns that fail to meet
these terms & conditions will be returned to the sender without approval, at the sender’s cost & will result
in credit forfeiture.
* ALL return shipments MUST be PREPAID & INSURED via a traceable carrier (e.g. UPS). Products
that originally shipped out via freight MUST be returned via freight to avoid warranty invalidation. ALFA
DOES NOT issue Call Tags & DOES NOT accept Freight Collect/COD Shipments. Please process your
returns and ship them to: ALFA International Corp. / Attn: (6 digit RA#) at our address noted below.
* Returns of unused/unopened products are subject to a 20% Restocking fee. Used merchandise Deemed
Un-saleable as New may be repaired & returned at our discretion in lieu of credit or replacement and
may be subject to additional restocking fees. Returns for repair are subject to an Evaluation Fee of FAQS & SCHEMAS
US $25.00 if the item is out of Warranty, Damaged, Altered, Unsanitary or found to be Defective as a
result of misuse.
